The Uttar Pradesh Age Certificate registration and application portal is now live at up-health.in. Uttar Pradesh Chief Minister Yogi Adityanath has confirmed that citizens can now apply for their Age Certificate digitally. You can submit your application via the Department of Medical Health and Family Welfare (DoMH&FW) website. Interested applicants may complete the online process at the official portal, up-health.in.

The DoMH&FW currently facilitates 9 digital services, including Medical Establishment Registration, Fitness/Illness Certificates, Disability Certificates, Immunization Records, Death Certificates, Family Planning Incentives, Medical Reimbursement claims, Medico-Legal Certificates, and Age Certificates.

This department provides these 9 services under the Janhit Guarantee Act. Residents no longer need to visit government offices in person, as they can now access these facilities from the comfort of their homes.
